Function & Responsibilities

Function and Composition

Safety and Health Commitee

• The Project Safety Committee shall be performed to comply with requirements from the Department of Labor and Employment as detailed in the Occupational Safety and Health Standards, and to assist in the accomplishment of safety objectives.

• A project safety committee as required by the company safety program will be organized and a regular monthly meeting will be held at the project site.

• The minutes of the meeting will be recorded by the project safety officer and distributed to all members. A copy of the minutes of the meeting shall be submitted to the Bureau of Working Conditions of the Department of Labor and Employment.

• The project Safety Committee shall establish coordination and communication of safety issues in the project.

• Provides necessary assistance to government inspecting authorities in the proper conduct of their activities such as the enforcement of the provisions of this Standard.

• Initiates and supervises safety training for the employees.

Safety & Health Committee Chairman – General Manager

• Promotes safety on the project through indoctrination and orientation of its project personnel.

• Develop applicable safety standards for the project, in accordance with the client’s safety program and program and government regulations.

• Maintain liaison with the client’s Safety department and others on matters of safety.

• Maintain overall responsibility for the first-aid facility and maintain required safety records.

Members – Supervisor / General Foreman

• Works in accordance with accepted safety practices and standards established by the employer in compliance with the provisions of this standard.

• Reports unsafe conditions and practices to the supervisor by making suggestions for correction or removal of accident hazards.

• Serves as members of the committee.

• Cooperate actively with the committee.

• Assist government agencies in the conduct of health and safety inspection or other programs.

 

Members – Nurses/First Aider

A registered nurse or a first aider, qualified by the Philippine National Red Cross, performs the following duties and responsibilities:

• Maintenance of the clinic in a professional and hygienic manner

• Performs first aid for injuries and illness according to accept medical practice.

• Refers more serious injuries and illness according to accept medical practice.

• Refers more serious injuries and illnesses to an off-site physician or hospital, as circumstances dictate.

• Keep records of all injuries and illnesses handled.

• Assist all employees during pre-employment and annual examination.

• Ensures the proper maintenance and confidentiality of employee’s chart, pre- employment, annual and exit medical examination.

• Set appointments for the treatment and record discharges from further treatment.

• Update and ensures the adequate supply of medicines.

Secretary – Safety Officer

• Coordinates the interpretation of local legislation and company policy pertinent to safety and health.

• Develops and maintains procedures and programs to create and reinforce awareness of safety and health, both on and off the job.

• Promotes injury-free operations by ensuring that appropriate safety rules are communicated to employees.

• Complies safety and health data and statistics and reports information to the site management.

• Recommends or conduct training as necessary to promote understanding of safety and health procedures and policies, and to combat hazardous practices.

• Perform daily safety inspection and reviews the result with the site supervision and ensures that all deficiencies are corrected.

• Refers significant process chemical exposures, regardless of employer, to the plant medical facility. (Referrals depends on the severity of exposure. In all cases, process chemical exposures should be reported to the nurse.

• Assist the safety manager injury/illness classification by providing factual, objective information.


Practices & Procedures

Indoctrination

• The Safety Manager and staff through project supervision will ensure indoctrination of each newly hired employee prior to any work assignment in accordance with the following rules, guidelines as mentioned below.
Safety Standard & Orientation

• The Safety Manager and staff will develop safety standards for the project and provide orientation for all supervisory personnel.

• The Safety Manager and staff will hold weekly safety meetings with the company’s personnel to review the Safety Program. He will encourage all levels of supervision to participate and improve the program. He will provide for instruction in first aid and accident procedures. Each foreman will conduct a weekly fifteen –(15) minute safety meeting prior to the commencement of the day’s work.

• The Safety Manager will take positive action to assure regular and effective meetings.

• The Foreman will be responsible for issuing individual safety instructions to all crewmembers along with work assignments.

• The Company supervisors will regularly monitor said meetings, comments and suggestions would be forwarded to Safety Department.

Safety Practices

• The Safety Department will maintain a surveillance of on-site working practices and conditions, including sanitary facilities, to assure conformance to good safety practices. He will notify the site Manager of unsafe conditions or practices.
Protective Equipment

• The company will issue to new employees a hard hat, safety glass, safety shoes and other personal protective equipment at the EPC&I / Site office. Approved safety
equipment as required by the company and local safety and health regulations shall be available at all times and strict enforcement of its proper use exercise by project supervision.

• The Safety Manager approves all safety equipment to be purchased by EPC&I.

Accident / Incident Reporting

• In the event of an accident on the Project, the injured employee’s supervisor will complete a “Supervisor’s Report of Accident. He will submit this form to the Safety Department who will immediately complete an “Employee’s First Report of Injury Form”. The Safety Department will process such accidents according to the severity. If it is a minor accident, a report shall be made and be distributed all personnel by their supervisors, so as to prevent recurrence. If it is a major accident, all concerned shall be notified. The company nurse attending an injured or sick employee shall comply with all regulations and submit reports at such time as may be required concerning the injured employees condition or treatment.

First Aid

• The Safety Manager will maintain responsibility for the first-aid program. The first-aid facility will be put into operation during the move onto the site. It will be operated for the duration of construction. A qualified individual will staff it, on duty at all times when labor is active on the job. Ambulance will be provided.
Record Keeping Requirement

• All safety record shall be compiled and a copy of which shall be submitted in a form required by the client or its safety representative.
Safety Hazards

• The Safety Manager is responsible for the implementation and administration of a project fire prevention program as required by the client or its safety representative.

CLOTHING REQUIREMENTS

Long pants and shirts shall be worn at all times. Sleeveless shirts, tank tops and jogging pants are not permitted.
Sandals, tennis shoes or any other soft cloth shoe shall not be permitted.
Loose clothing such as ties, scarves, dangling belts, loose sleeves, long flapping shirttails and skirts shall not be worn when in construction area.
Jewelry such as rings, watches, bracelets, necklaces and any earrings more than 1/2/ inch shall not be permitted to be worn when in construction area.
A hard hat or hair net shall protect hair that could come in contact with, or be caught in machinery.


General Safety Rules

 

• PERSONAL PROTECTIVE EQUIPMENT
Approve hard hats meeting specifications contained in American National Standards Institute, Z89.1-1981 and/or Z89.2-1971, SHALL BE WORN AT ALL TIMES.

Proper eye and face protection shall be worn when engaged in such operations as welding, burning, chipping, grinding, handling chemicals, and pouring concrete. SAFETY GLASSES WITH SIDE SHIELDS WILL BE THE MINIMUM TYPE OF EYE PROTECTION AND WORN AT ALL TIMES.

Approved safety shoes shall be worn at all times. OSHA approved hearing and respiratory equipment shall be worn when required. The selection, fitting and maintenance requirements of OSHA shall be met. All employees shall wear full body harness when working five feet or more above the ground when no other type of fall protection is provided. The lanyard shall be securely attached to the employee 100% of the time and shall allow a maximum fall distance of five feet. Full body harness shall also be worn when working out of extensible and articulating boom platforms or suspended scaffolding.

All employees operating any motor vehicle and any equipment with rollover protection structures while on the project shall wear seat belts.
Safety nets shall be provided when work places are more than 25 feet above the ground where the use of other fall protection devices is impractical.

• SIGNS, SIGNALS AND BARRICADES
Signs, signals and barricades shall be visible at all times where a hazard exists.
Signs, signals and barricades shall be removed when the hazard no longer exists.

• RIGGING EQUIPMENT
The Equipment Supervisor prior to initial use shall inspect rigging equipment on a monthly basis to ensure safety. Records shall be kept in the Company’s site office on each of these
inspections.
Damaged rigging equipment shall be removed from service immediately.

• COMPRESSED GAS CYLINDERS
Compressed gas cylinders shall be secured in an upright position in a bottle cart at all times. The Owner and the Contractors Project Site Manager must approve location of cylinder storage areas.

“No Smoking” signs shall be posted at storage areas and sign shall clearly indicate contents of cylinders.
In addition, EPC&I shall follow all applicable rules and regulation of the client concerning safe management and handling of compressed gas cylinders.

• HAND AND POWER TOOLS
All hand and power tools, whether furnished by the EPC&I or by the employee shall undergo inspection from the Client’s safety representative to ensure compliance of Client’s safety standard.
Tool keepers shall not issue or permit the use of damaged or unsafe hand or power tools.

Electric power tools shall either be double insulated or grounded.
Pneumatic power tools shall be secured to the hose or whip by some positive means.
Only properly trained employees shall operate. The tool keeper shall maintain
certification records, employee certification cards, or equivalent certification documentation. All grinding machines shall conform to all OSHA and ANSI
requirements.
Cords, leads and hoses will be kept off the ground at seven feet or whatever height is necessary to be protected from traffic.

• SCAFFOLDS
Footings or anchorage for any scaffold shall be rigid and capable of carrying the maximum intended load without settling or displacement.
No unstable objects such as concrete blocks shall be used to support scaffolds or planks.

Any part of a scaffold weakened or damaged shall be repaired or replaced immediately.

Guard rails and toe-boards shall be installed on ALL open sides and ends of scaffold platform ten (10) feet or more above the ground or floor. Scaffolds 45” or less in length or width shall have standard guardrail on all open sides and ends 4’ higher above the ground or floor. Guardrails shall be 2” x 4” construction or the equivalent, 42” high with a mid-rail and 4” high toe-board. Vertical supports shall be spaced at 8’0 maximum spacing.

All scaffold planking shall be free of knots and cracks and shall completely cover the work platform. All planking shall be scaffold grade.
Scaffold planks shall be laid tight, cleated at both ends and overlapped a minimum of 12”. Overlaps shall be nailed or bolted to prevent movement and shall occur directly over scaffold supports.

Safe access shall be provided to the scaffold platform. Specifically, a ladder with a safe means of access to the platform from the ladder.
Bamboo or coco lumber scaffolding shall not be used.

• CRANES
All cranes and derricks if by chance shall be required to perform a specific job shall be certified as being in safe operating condition by the Equipment Supervisor of EPC&I in accordance with the client’s safety manual and or with the conformity of the client’s safety representative prior to using the crane on site. This certification shall be maintained in the equipment files. The Safety Manager or Authorized Representatives shall certify all crane operators. Certification records should be maintained in the equipment files. All crane operators shall have medical physical exams on a yearly basis. The swing radius of the crane shall be barricaded. Hand signals prescribed by ANSI shall be posted at the operator’s station.Manufacturer’s specifications shall be observed.Equipment shall not be operated where any of the equipment or load will come within 15 feet of electrical distribution or transmission lines.

No person shall ride the headache ball, hook or load being handled by the crane.
Equipment shall not be lubricated while in use.
Rated load capacities, recommended operating speeds, special hazard warning, specific hand signal diagram, and special instructions shall be visible to the operator while he is at the control station. Employees shall not be allowed to work under the load of cranes. Crane lifts shall be categorized into three types

• Engineered Lifts
All one crane lifts of 70% of the crane’s capacity and above
All two or more crane lifts

• Permit Lifts
Loads 5 tons and above in weight
Loads above 50% but below 70% of the capacity chart
All lifts being made with a jib
All lifts being made in operating units over lines and equipment
Any loads that fall within the tipping part of the capacity chart.

• Non Permit Lifts
Loads less than 5 ton in weight
Loads that are less 50% of the crane’s capacity
Any loads that fall within the structural part of the capacity chart.
Necessary permit and rigging plans shall be accomplished, submitted and approved before any lift shall be made.

• EQUIPMENT AND MOTOR VEHICLES
All equipment must be inspected daily before use by the operator. Formal inspections must also be made at thirty (30) day intervals with proper documentation maintained in the equipment files.

Defective equipment will be repaired or removed from service immediately.
All operators of construction equipment should be certified. And shall carry an EPC&I Equipment operator license. Copies of the certifications shall be maintained in the safety files. All equipment with obstructed view to the rear must have a reverse signal alarm audible above the surrounding noise level or must have flagman.
All cracked and broken glasses shall be replaced before bringing vehicles on the jobsite. If glass is broken or damaged on site and if damage is severe enough to cause a potential safety problem, the machine will be stopped until such damage has been repaired.

Vehicles used to transport employees shall have seats firmly secured and adequate for the number of employees to be carried and all passengers should be properly seated. Standing on the back of moving vehicles is prohibited.
Locations for storage of all fuels, lubricants, starting fluids, etc. shall be approved by the Client and or its representative.

• ELECTRICAL
All electrical work, installations, and wire capacities shall be in accordance with the pertinent provisions of the National Electrical Code, ANSI, and OSHA.
EPC&I to ensure that tools and equipment are still safe, practice a monthly electrical tools and equipment check. EPC&I practice the lock-out, tag-out and check procedure before commencing work on presumed energized equipment.
All temporary power panels shall have covers installed at all times.

• LADDERS
The use of ladders with broken or missing rungs or steps, broken or split rails or other defective construction is prohibited.
Ladders shall extend no less than 36 inches above landing and be secured to prevent
displacement.

Portable ladders must be equipped with non-skid safety feet.
Wooden ladders shall not be painted.Only client and or their safety representative approved ladders shall be used.

• EXCAVATIONS
Prior to opening any excavation or trench, the Client and or its representative shall be notified and excavation permits obtained. Excavations and trenches shall be inspected by a competent person daily, after every rainfall, as soil conditions change, and as needed throughout the shift. If there is evidence of possible slides or cabins-ins, indications of failure of protective systems, hazardous atmosphere, or other hazardous conditions, necessary safety precautions must be taken before work may proceed.

Material excavated shall be stored at least two feet from the edge of the excavation or trench or shall be barricaded to prevent material from falling into the excavations.
All trenches and excavations shall be properly barricaded to prevent persons from walking into them.

All walkways or ramps crossing over excavations shall be securely fastened and equipped with standard guardrails.

• CONFINED AREAS OR SPACES
Employees required to enter confined areas or space shall be instructed as to the nature of hazards involved, necessary precautions to be taken and in the use of protective and emergency equipment required.

Before employees are permitted entry into any confined area or space, the atmosphere within the space shall be tested to determine, the oxygen level and concentrations of flammable vapors, gases, toxic contaminants. An employee shall be stationed outside the confined area to maintain communication with those working within and to aid them in an emergency.

Employees required to enter confined areas or space shall be instructed as to the nature of hazards involved, necessary precautions to be taken and in the use of protective and emergency equipment required.

Before employees are permitted entry into any confined area or space, the atmosphere within the space shall be tested to determine, the oxygen level and concentrations of flammable vapors, gases, toxic contaminants. An employee shall be stationed outside the confined area to maintain communication with those working within and to aid them in an emergency.

• HOUSEKEEPING
During the course of construction, all debris and scrap materials shall be kept away from the work area.

The client and or their representative shall provide containers for the collection and separation of waste trash and metal. Garbage and other wastes shall be disposed of at frequent and regular intervals.
EPC&I shall notify the Client of any hazardous waste it will generate while on the project.
EPC&I’s site office shall be maintained according to Plant standards
